This page explains what Sheen Dental asks for through this website, what we do with it and what we keep private.

What the contact form collects

The form asks for your name, email, phone number, how you would like us to reply, the time that suits you and a short message. We use it to answer you and to find a time for your visit.

Please keep health details out of messages

A contact form is not the place for your health history or symptoms. If something is urgent, call us. Health history and consent forms are completed in our patient portal or on paper at the office.

Your patient records

We keep your records private and share them only as the law allows. Our notice of privacy practices, given at your first visit, explains your rights and how to use them.

Who sees your information

Only the staff who need it to answer you or care for you. We do not sell your information. We share it with a lab, a referring or receiving dentist, or your insurance plan only when it is needed for your care or your claim, or when the law requires it.

Your choices

You can ask to see, correct or delete a message you sent us, and you can ask us to stop contacting you by email or text. Contact us and we will tell you what we can do.
